Passports


The Roxbury Public Library is a State Department certified Passport Facility. You can apply for a brand new US Passport here, as well as renew an existing passport. We also take passport pictures on site! Photocopying of IDs and other documents accompanying your application is complimentary :)

Apply in Person


If

  • you are a first time applicant OR
  • your previous passport was issued before you were 16 years of age OR
  • your previous passport expired more than five years ago OR
  • your previous passport is lost or stolen 

then you DO need to visit a passport acceptance facility and apply in person. Speed things up and fill out the DS-11 ahead of time!

If you or any member of your family are in this category, please make your appointment using the calendar below for everyone in the party, even if some of you only need a picture. One appointment can cover everyone in the family. 

Need a Picture only?


If you are

  • an adult renewing a 10-year passport AND 
  • you still have your old passport AND
  • your passport has not been damaged AND
  • your name hasn't changed except due to marriage

then you do not need the services of a passport facility. All you need to do is fill out and follow the instructions on the DS-82 Passport Renewal Form and mail everything yourself.

First Time Applicants - you must apply in person if:


  • You are a first time applicant
  • Your previous passport was issued before you were 16 years of age
  • Your previous passport expired more than five years ago

* Minors must be present as well as all parents listed on the birth certificate in order to execute the application. One parent should accompany minors ages 16 to 17 unless the minor has a valid driver’s license (not a learner’s permit)

 
To obtain a U.S. passport, you will need each of the following items:

  • DS-11 Application
  • Proof of citizenship
  • Proof of identity
  • Photograph (we take pictures if you need it–$10 cash per person)
  • One check or money order per application
  • $35 cash per application for the execution fee

Proof of Citizenship


Please be prepared to surrender these documents. They will be returned to you by the Department of State approximately 1 week after you receive your passport.

U.S. Born***

U.S. born persons need an original birth certificate, listing both parent’s names, with a raised seal and filing date from the Bureau of Vital Statistics or city of birth. Hospital-issued birth certificates and birth certificates issued by the Hudson County Clerk’s Office or the city of Jersey City are not acceptable. If your birth certificate was issued by the Hudson County Clerk’s Office or the city of Jersey City, you must obtain a birth certificate from the New Jersey Bureau of Vital Statistics in Trenton (609-292-4087 or 609-633-2860).

Children under the age of 16

All minors must be present. Both parents must execute the application and provide ID. Children under the age of 16 must submit a long form birth certificate, which includes both parents’ names. This is required even if the child has previously had a passport.

Foreign-born***

Foreign-born persons must submit a naturalization certificate. Foreign-born children of naturalized parents may apply for a U.S. passport, provided the child was under the age of 18 when the parents became naturalized. In that case the child must submit their foreign birth certificate with the parents’ names alongside the parents’ naturalization certificate.


***There are other scenarios listed in more detail on travel.state.gov for adopted indviduals, persons born outside of the US to one or more parents or US citizens, etc. Please do your due diligence by checking over travel.state.gov to ensure that you bring the required documents for your circumstances. 


Proof of Identity 

(All documents must be originals. No copies accepted.) One of the following:

  • Valid State driver’s license
  • Naturalization Certificate
  • Prior US passport
  • Government employee ID
  • Official Military card
     

Photographs

Present 1 passport photograph taken within the past 6 months. Photo must be 2×2 inches in size (no exceptions). It should be front view with a white or off-white plain background. The applicant must be the only person in the photo. ​

***The library takes passport photographs. If you’d like to have your photos taken at the time of application, let us know. The fee is $10 per person.


Minors


Children under the age of 16

All minors must be present. Both parents must execute the application and provide ID. Children under the age of 16 must submit a long form birth certificate, which includes both parents’ names. This is required even if the child has previously had a passport.


**If one of the parents cannot be present for the application’s execution, that parent must fill out and sign Form DS-3053 in front of a Notary. The applying parent must show this form, as well as a copy of the front and back of the non-applying parent’s valid ID during the execution.

 

Children ages 16 to 17

One parent should accompany minors ages 16 to 17 unless the minor has a valid driver’s license (not a learner’s permit)


Fees and Processing Time


You will need to make TWO payments: one to the U.S. Department of State (separate check per applicant) and one (cash only) payment to the Roxbury Public Library. All fees are set by the Department of State and are non-refundable. All passport facilities charge the same fees.

1. Passport Fee

This fee must be paid by check or money order payable to “U.S. Department of State.” One check/money order per application. On the bottom left side of the check/money order, please write the applicant’s full name and date of birth.


Passport Book Fee for Routine Processing (10-13 weeks) 

There are serious delays in issuing passports during the Covid-19 Pandemic. Do not expect to get your passport soon!

  • Age 16+: $130
  • Under 16: $100


Passport Book Fee for Expedited Processing (7-9 weeks)

  • Age 16+: $130+ $60 = $190
  • Under 16: $100 + $60 = $160 


2. Facility Execution Fee

Cash only in the amount of $35 per applicant. Please note that if you choose to take pictures on site with us, we charge an additional $10 per person.


The U.S. Passport Card


The Department of State began offering a U.S. Passport Card in the summer of 2008 in response to the needs of border resident communities for a less expensive and more portable alternative to the traditional passport book.  The card has the same period of validity as a passport book: 10 years for an adult, five for children 15 and younger.

The passport card facilitates entry and expedites document processing at U.S. land and sea ports-of-entry when arriving from Canada, Mexico, the Caribbean and Bermuda. The card may not be used to travel by air. Otherwise, it carries the rights and privileges of the U.S. passport book and is adjudicated to the exact same standards.

The Passport Card costs $30 for applicants 16+ and $15 for applicants under 16. It can be purchased alone or in addition to a passport book. The facility execution fee applies whether you are getting a card alone or a card in conjuction with a book.
